Note: Assessment is due by the 15th of March, June, September and December.

Note: All questions on this questionnaire were taken
either directly from the Georgia EPD inspection form that will be used
during an inspection or from the items recently cited during the EPD inspections
of laboratories at sister institutions (UGA & Ga. Tech.).
1. All waste containers are properly labeled (40 CFR 262.34(c)(1))

Any item in which No was the response should be remedied. The
Environmental Programs staff members can be called on to assist you in

NOTES
*Cited as a violation during
the 1997 EPD inspection of Kell Hall and NSC. Existing problems with these
items would constitute a more serious violation as it would be a repeat
violation.
1 Inspectors consider unlabeled
containers as being unknown, thus unusable. Unless personnel can identify
the contents as usable materials, inspectors will define the containers
as waste because they meet the definition of a waste (…unusable or discarded
material.). As such, all waste regulations apply to said container.
2 Listed Hazardous chemicals
beyond their expiration dates meet the definition of a waste in 40CFR262
and must be handled according to waste regulations unless it is shown that
this material is still in use.
3 Containers that are not
in good condition are considered by inspectors as holding chemicals that
are not used and are no longer usable thus meeting the definition of a
waste unless it can be shown that the material is still in use. U
and P listed chemicals are considered to be a waste if the product is “off-spec”.
4 Chemicals in storage that
are inherently waste-like or have no future use are considered waste.
